A few weeks back, I posted a flyer where I listed out the various forms hospitals use when committing a mentally ill patient. I recently stumbled across another form number for cases where a patient wants to transfer to another facility. This is a right they have, are never told about, and something the hospitals loathe because it means poaching from another facility. See the added Form 1007 to the attached flyer.
I've also included the Form 1013 for those who have never seen the elusive document. This is the Form filled out by a qualified individual authorized to find that a person meets emergency inpatient hospitalization criteria under O.C.G.A
. §37-3-1.
